Street Outreach Case Worker

JOIN’s Outreach team members conduct in-person street outreach, prioritize relationships and connections, with an emphasis on building trust, supporting individuals’ strengths, and creating short and long term goals toward participant stability.

Outreach is defined as going to parks, under bridges, bi-ways, bike paths, and other places not meant for habitation where people camp in order to provide information and vital resources towards housing placement. Effective outreach often requires working flexible non-traditional hours, including some evenings and weekends.

JOIN’s Outreach team uses a relationship model to develop and support case management plans, providing housing navigation resources within Multnomah County, in support of each individual obtaining stable long term housing, while demonstrating genuine care, dignity and respect for each person we encounter along the way.
