JOIN leads a multi-agency Mobile Permanent Supportive Housing Team (MPSH) that delivers support services to people living in site-based and scattered-site Permanent Supportive Housing (PSH) units.  

Compensation:

Salary: $46,500 – $57,600

Position Overview

JOIN is hiring a Mobile Permanent Supportive Housing (MPSH) Case Manager. As an interdisciplinary team, the MPSH program prizes our work on shared case management responsibilities with behavioral health support, physical healthcare, peer support, and culturally specific and responsive services. The case managers provide housing stabilization support for people recently housed with permanent supportive housing vouchers, both through internal and external referrals.  We work to empower participants to maintain permanent, independent housing through acquiring income, promoting physical and emotional health, and building community.  MPSH workers need to be able to build authentic relationships and work effectively with people experiencing homelessness, regardless of their backgrounds, personal histories, household composition, and be willing and able to engage people on the streets, in their homes, in community settings, and in our service facility. MPSH participants are generally high acuity, with 0-30% income, could be families, individuals or couples. Folks immersed in Indigenous, Vietnamese, Eritrean, and Latinx communities are strongly encouraged to apply.

Essential Duties and Responsibilities

  • Provide home visits to recently housed people in support of their self-determined housing stability plan.
  • Support households in maintaining compliance with housing voucher requirements, certifications, and inspections
  • Coordinate and support referrals to services within the MPSH team and with partner agencies, including disability benefits acquisition, employment assistance, domestic violence providers, behavioral health, and workforce training (internal and external referrals).
  • Provide support and advocacy around households’ engagement with physical and mental health resources, the Department of Human Services, and the criminal justice system.
  • Coordinate and/or provide transportation to appointments, interviews, events, and other functions.
  • Apply principles of harm reduction, trauma-informed care, and strengths-based approach; utilize tools such as eco mapping to assist in these efforts.
  • Openness to skills development training.
  • Build and maintain strong relationships with resident services coordinators, property managers, and management companies to support housing stability and to create avenues for advocacy when a household’s tenancy is at risk.
  • Coordinate with emergency food providers to obtain and deliver food boxes.
  • With the coordination of housing placement workers, provide delivery of furniture and household goods to newly housed individuals.
  • Work with internal staff and external agencies to assume households on caseloads from both adult and family systems of care
  • Navigate school systems/family resources

Working with Staff  

  • Work in conjunction with the MPSH Team to attend regular meetings, manage partner agency relationships, case staff with the team, and manage shared team budget allocation in supporting participants.
  • Maintain narrative and required data regarding services provided in a timely manner
  • Engage respectfully with all staff and be willing to collaborate, support each other, and approach conflict constructively
  • Engage in JOIN’s equity and anti-racist work at the full-staff, program-specific, and individual professional level.
  • Apply an equity lens to work with staff, service users, volunteers, donors, and community partners.

 REQUIRED Education, Experience, Qualities, or Specialized Training

  • Demonstrated respect for the value of diversity and equity; an ability to work with people from all backgrounds in an inclusive and meaningful way. 
  • Demonstrated experience providing culturally specific and responsive services 
  • Experience building trusting relationships and collaborative efforts with diverse individuals and organizations.
  • Commitment to non-violence and holistic approaches.
  • Must be self-motivated and have a demonstrable ability to work well in a fast-paced, structured, team-driven environment
  • Ability to balance competing priorities and demands from people who are in crisis
  • Demonstrated commitment to person-centered and responsive service delivery
  • Ability to work with or learn computer data entry programs and familiarity with Office Suite software.
  • Maintain accurate and timely records of services provided.
  • Must own or have access to a personal vehicle for daily use on the job
  • Must have a valid driver’s license and insurable driving record upon hire.
  • Must be able to work occasionally in the evenings and on weekends

PREFERRED (not required) Education, experience, qualities, or specialized training:

  • Experience working with people experiencing homelessness or other similar personal challenges.
  • Proficiency in one or more of the following languages: Spanish, Vietnamese, Chinese simplified, Chinese traditional, Russian, Ukrainian, Somali, French (West African), Swahili, Arabic, Romanian, Nepali, Chuukese, Lao, Tagalog, Cambodian, Japanese, Korean, Karen, Marshallese, Thai, Amharic, Burmese, Tigrinya, Farsi.
  • Familiarity with ServicePoint HMIS systems

Specific physical and mental performing elements:

Ability to lift and carry 50 lbs.; position involves regular moving of food boxes, furniture into and out of vehicles and apartments.

Equipment use:

Be well versed in using computers, G-Suite, Microsoft Office, and smartphones with varying apps used to perform daily tasks
